社団法人京都府薬剤師会 様の「薬のプロフェッショナル 在宅医療への取り組み」ムービーを制作させていただきました。近い将来、超高齢化社会を迎え、在宅医療が増えていくことが予想されている中で、多業種連携 の一員として業務に取り組んでおられる薬剤師さんの業務を紹介するための映像です。

退院カンファレンスによる病院医療チームから在宅医療チームへの申し送 りなど、一人の患者様が安心してご自宅で在宅療養ができるように、 薬剤師・医師・歯科医師・看護士・ケアマネジャー・介護スタッフ・医療スタッフ・ ソーシャルワーカーなどの多業種に携わる方々が連携をとりながら見守る取り組みを紹介しています。

ICommitment to professional in-home healthcare This video, “Commitment to professional in-home healthcare”, was produced for the Kyoto Pharmaceutical Society Institute. In the near future Japan will face the challenge of caring for an increasingly elderly population. As such, it is expected that in-home healthcare practice will see a rapid expansion. This video introduces the role of pharmacists in cooperation with a multi-disciplined group of healthcare professionals. This healthcare team, consisting of doctors, nurses, care managers, pharmacists, dentists, social workers, and medical staff, collaboratively monitor the health and well-being of each patient from the safety and comfort of their own homes.
